In application/x-www-form-urlencoded format (HTML forms), plus signs represent spaces. This decoder converts both + and %20 to spaces. Literal plus signs should be encoded as %2B.
Double encoding occurs when already-encoded text is encoded again (%20 becomes %2520). If your result still shows percent sequences, run it through the decoder again. Be careful not to over-decode.
This usually happens when the original encoding was not UTF-8, or when decoding binary data (like images) that was never meant to be readable text. The decoder is working correctly, but the data has a different format.
